We found out that the QSO end date of one of the certificates was exceeded. Thus the certificate cannot be used for confirmations any more. User needs to request a new certificate as the QSO end date cannot be modified for existing certificates.
Hint: Leave date of last QSO empty while requesting the certificate. QSO end date will then be the expiry date of the certificate itself.

For visual aid of this we created #2437 which adds some font highlights to LoTW cert page to help the user identify the issues. The dasboard banner and manual LoTW sync output have been modified accordingly.
